First Coast Advantage (FCA) Member Contact Us Tab Added on Website:

 

Effective June 29, 2010, FCA has added a Member Contact Us Tab on the FCA website. Members will now be able to send FCA an e-mail when they want to request, update or change their information such as:

  • Change their PCP                                                                    

  • Change their Address

  • Request New ID Cards

  • Request New Member Materials

  • Request Provider Information

  • Request Benefit Information

  • Other Request
